Complementary Course.
Grade 11 (NSSCO) & Grade 12 (NSCAS)
– Grade 11 (new curriculum students who want to improve results) – Former Grade 10, 11, and 12 students (old curriculum) – Grade 12 (AS Level) students who need to improve their Mathematics, Physics, Chemistry and Biology AS level results

- Please read the enclosed information leaflet before completing this form.
- Complete the form in full and answer all the questions.
- Your name(s) must be according to your birth certificate.
- Where a choice is given, mark only the appropriate response with an X.
INCOMPLETE AND FAXED APPLICATION FORMS WILL NOT BE PROCESSED.
Please note that nobody will be allowed to enter the premises without a face covering. We will have face masks as well as face shields available at a cost of N$30.00 for purchase.
